Due to demographic changes and ever-evolving treatment options, the number of oncological patients is steadily increasing, which is also reflected in prehospital care. Considering the often lengthy suffering of these patients, an understandable and empathetic communication is essential, for which the SPIKES model is well-suited. Pain, dyspnea, fever, infections and bleeding are common reasons for consultation, requiring professional expertise and specific actions. Furthermore, central venous catheters such as a port catheter or peripherally inserted central catheter (PICC) represent unique considerations for oncological patients. The use of these catheters can be especially beneficial in cases of poor venous access. The decision regarding treatment limitations poses a particular challenge in prehospital settings as complex findings must be assessed in a very short time. Written advance directives and discussions with patients and their families can provide valuable support.
