<p>The attack on the Magdeburg Christmas Market on 20&#xa0;December&#xa0;2024, resulted in a&#xa0;complex mass casualty incident (MCI) with over 299 injured individuals, necessitating a&#xa0;coordinated emergency medical response across multiple hospitals. The University Medical Center Magdeburg (UMMD) implemented its MCI management plan based on the recommendations of the German Federal Office of Civil Protection and Disaster Assistance (BBK) and the German Working Group for Hospital Emergency Planning (DAKEP). Structured alarming, the organization of the Hospital Incident Command System (KEL), and targeted patient distribution enabled efficient management of the situation. To systematically analyze the medical response following the attack, an anonymized survey was conducted among the involved hospitals. The results indicate a&#xa0;homogeneous distribution of injury patterns, with a&#xa0;high prevalence of blunt trauma and fractures. The treatment procedures in the first 12 h were characterized by comprehensive radiological diagnostics, targeted initial surgical interventions, intensive medical care, and psychosocial support. Capacity constraints did not play a&#xa0;significant role; however, challenges in information transmission and resource allocation were identified. The analysis reveals that the pre-established structures functioned effectively, although certain areas reveal potential for optimization. In particular, real-time communication between treatment areas and incident command, as well as psychosocial emergency assistance, proved to be essential. The insights thus obtained provide valuable approaches for the further development of hospital alarm and emergency response plans for future large-scale emergencies.</p>
