The infection of a diabetic foot is a dramatic complication in people with diabetes mellitus and a septic foot infarction is a severe variant in the sense of necrotizing fasciitis. Early detection, timely diagnosis and prompt treatment are crucial for limb preservation and survival. The cornerstones of sepsis control are immediate adequate anti-infective treatment and surgical intervention. The surgical principle involves urgent debridement of all avital (gangrenous) tissue, open wound drainage and preservation of vital tissue for later (partial) reconstructive wound coverage. After infection control, early revascularization is necessary in cases of proven critical ischemia. Only then should a decision be made on whether a (minor) amputation is necessary.
