Traumatic injuries of arteries in the extremities are the most frequent cause of amputations in young people. Due to the visible bleeding, sharp vascular injuries are rarely overlooked and with adequate compression are only life-threatening in exceptional cases. The correct diagnostics are challenging, especially in blunt vascular injuries without externally visible bleeding. In the latter case, in addition to an often expanding size of a hematoma and the resulting compression, varying degrees of ischemia are the most important. Computed tomography (CT) angiography is the diagnostic tool of choice, which is nowadays widely available and typically initially used as a polytrauma spiral. Arterial reconstruction should preferably be carried out autologously using an interposition, less often as a patch plasty or using an end-to-end anastomosis. A fasciotomy is mostly recommended even during the initial intervention, especially in the case of ischemia over several hours and clinical signs of a compartment syndrome.
