Smoking is the leading preventable risk factor for lung cancer and continues to affect prognosis, treatment tolerance and comorbidity after cancer diagnosis. Nevertheless, structured smoking cessation support remains insufficiently implemented in oncology care.
ObjectivesThis narrative review summarizes current evidence on the effectiveness of smoking cessation in patients with cancer, with a focus on lung cancer, and analyzes biological, clinical and health economic arguments supporting its implementation as standard therapy.
Materials and methodsA narrative review based on PubMed-listed original literature from 2010 to 2026 was performed. Particular emphasis was placed on systematic reviews, meta-analyses, guidelines and selected recent original studies addressing survival, recurrence, second primary malignancies, treatment interactions, intervention efficacy and health economic outcomes.
ResultsSmoking cessation at diagnosis or shortly thereafter is associated with improved overall survival in lung cancer. In patients with cancer, combined behavioural and pharmacological cessation interventions show the most consistent benefit for abstinence. Additional evidence indicates reduced risk of second primary lung cancer and favourable economic effects of integrated cessation programs.
