<p>Hospitals are the most common place of death in Germany. Nevertheless, the quality of care in the dying phase provided in this setting is often considered inadequate. Major challenges include the delayed recognition of the dying phase and the lack of adherence to evidence-based guidelines. Guideline-based care requires the early identification of the dying phase, regular and systematic assessment of symptoms and distress, including their appropriate treatment, and the discontinuation of all medical, nursing, and therapeutic interventions that do not primarily serve the goal of symptom relief. Informal caregivers play a&#xa0;key role in the care of dying patients and should be actively involved and supported through repeated information sharing, opportunities for participation, and access to support services. Structural measures such as trainings, standardized operating procedures (SOPs) and information materials can contribute to improving the quality of care in the dying phase in hospitals.</p>
