Antimicrobial Stewardship in der Onkologie

Patients in oncology are particularly vulnerable to infections due to immunosuppression caused by the underlying malignancy and its therapy. As a result, broad-spectrum antibiotics are frequently administered empirically. However, this approach carries significant risks, including the emergence of resistant pathogens, adverse drug reactions, and disruption of the physiological microbiota. Antimicrobial stewardship (AMS) aims to promote the rational use of antimicrobial agents in order to preserve their efficacy and minimize the development of resistance. In the oncological setting, this means close interdisciplinary cooperation (e.g., between hematology, oncology, and microbiology), differentiated risk assessment and consideration of individual factors such as neutropenia or transplant status. Evidence suggests that AMS programs can improve the quality of care in this vulnerable patient population without compromising patient safety. This overview is based on a selective literature search in the PubMed database, conducted jointly by specialists in hematology, oncology, and infectious diseases. The review considered original research articles, systematic reviews, and clinical guidelines focusing on AMS strategies in immunocompromised patients with cancer. Careful microbiological diagnosis, early re-evaluation of empirical therapy, and structured implementation of guidelines are essential components of successful AMS in oncology.
