Oligometastasierung: das neue Paradigma

The term “oligometastasis” was coined over 30 years ago. While it initially relied mainly on retrospective studies and anecdotal evidence regarding the positive prognostic role of local therapies in patients with oligometastatic disease, a large number of randomized phase II trials have been published in the meantime.
ObjectiveThis article aims to provide a historical perspective and an overview of the current literature on local therapy for patients with oligometastatic disease.
MethodsThe work comprises a semi-structured literature review.
ResultsAt least 15 randomized controlled trials studying the role of metastasis-directed therapy in patients with oligometastatic disease have been published. The overwhelming majority of these trials demonstrated a clinically meaningful improvement of progression-free and, in some cases, overall survival, with minimal additional toxicity. Stereotactic radiotherapy was the predominant local treatment modality. Results from confirmatory phase III trials are pending.
ConclusionMetastasis-directed therapy improves the prognosis of patients with oligometastatic disease. The choice and use of local and systemic therapy in patients with oligometastatic disease should be discussed in multidisciplinary tumor boards.
