Advances such as targeted and immunotherapies hold promise for improving survival and overcoming treatment resistance.</p> Objective <p>This manuscript reviews recent clinical trial data and real-world analyses on MCL treatment, highlighting the shift away from autologous stem cell transplantation (ASCT) and the emerging role of targeted agents and immunotherapies.</p> Materials and methods <p>We analyzed results from key clinical trials, including the TRIANGLE, ECOG-ACRIN 4151, ENRICH, and SHINE studies, as well as real-world data. Emphasis was placed on treatment outcomes, progression-free survival (PFS), overall survival (OS), and treatment-related toxicities.</p> Results and conclusion <p>The integration of BTK inhibitors, reduced reliance on ASCT, and robust maintenance strategies have transformed first-line MCL treatment paradigms. In relapsed/refractory settings, various generations of BTK inhibitors and immunotherapies hold great promise for sustained disease control, improved long-term survival, and enhanced quality of life, offering novel modes of action that can overcome resistance mechanisms.</p>
