<p>The assessment of symptoms based on patient-reported outcomes is considered the gold standard for successful multidisciplinary symptom management. A&#xa0;suitable tool is needed to assess symptoms in a&#xa0;focused and structured manner. At HOCH Health Ostschweiz, no standardised symptom assessment has taken place to date. As part of a&#xa0;practical project, a&#xa0;patient-reported outcome-based symptom assessment tool has been developed, implemented and made accessible to the multidisciplinary team. The next step is to digitise the system.</p>
