Terminale Herzinsuffizienz: Therapie mit „Kunstherz“

Treatment of end-stage heart failure remains a growing challenge in modern cardiology and cardiac surgery. Although heart transplantation continues to be considered the most effective therapy, the shortage of donor organs significantly limits its availability. Left ventricular assist devices (LVADs) have therefore become an established option both as a bridge to transplant (BTT) and as a permanent treatment strategy (destination therapy, DT), with survival rates—particularly among younger patients—that approach those achieved with transplantation. This review summarizes the current evidence, technical developments, as well as surgical and postoperative aspects of LVAD therapy, and further discusses clinical, ethical, and economic challenges.
