Anorectal malformations (ARM) affect approximately 1 in 5000 newborns and require specialized long-term care. Appropriate diagnosis includes VACTERL (vertebral defects, anal atresia/imperforate anus, cardiac defects, tracheoesophageal fistula, renal anomalies, limb abnormalities) screening, imaging studies, and evaluation of the sacrum and spinal cord. The Krickenbeck classification enables precise categorization and prognostic assessment. While surgical correction should be performed in specialized centers, long-term postoperative care by pediatricians is crucial. Structured bowel management programs demonstrate significant improvements in continence rates and quality of life, while reducing emergency department visits and healthcare costs. A multidisciplinary approach with prokinetic medications, prevention of chronic constipation, and psychosocial support are essential for good functional outcomes. Even with persistent incontinence, therapeutic options exist to achieve social continence.
