<p>An association between COVID-19 and coagulopathies with high levels of several biomarkers of thrombosis, that impact disease severity and prognosis are reported. The purpose of this study was to investigate the procoagulant pattern and fibrinolytic activity among a cohort of COVID-19 Egyptian patients, and the impact of the studied parameters on patients’ mortality. Fibrinogen, D-dimer, FDPs, and soluble fibrin monomers were assayed using fully automated coagulation analyzer in 73 COVID-19 patients with variable clinical severity (mild, moderate, severe, critical) to demonstrate activated coagulation and fibrinolysis. Naturally occurring anticoagulants, fibrinolysis activators, and inhibitors using ELISA assay were studied with their impact on patients’ mortality. All of these results undergone statistical analysis. High D-dimer, FDPs, and fibrinogen were observed in patients with severity varying from moderate to critical disease in comparison to mild cases, while SFM were elevated irrespective of severity. Higher concentrations of TAT, t-PA, and PAI were observed in moderate, severe, and critical cases. Lower insignificant levels of AT, PC, and free PS activity were in non-survivors than survivors, while elevated t-PA and PAP concentrations were among non-survivors. Although some biomarkers of COVID-19-associated coagulopathy, including high levels of fibrinogen and D-dimer were recognized, other markers reflecting the activation of coagulation such as TAT or activation of fibrinolysis such as PAP can be introduced to assess thrombotic risk in certain situations where the conventional markers are inconclusive.</p>
