Neurotrophic keratopathy patients after minimally invasive corneal neurotization treated with RhNGF eyedrops: a case series

To assess whether recombinant human nerve growth factor (rhNGF) eye drops used in neurotrophic keratopathy (NK) patients after minimally invasive corneal neurotization (MICN) surgery can accelerate the process of reinnervation and shorten the patient’s postoperative recovery time.
MethodsThe authors highlight 5 cases that have all undergone MICN and been treated with rhNGF eyedrops. Cases include: 4 cases that have been over 1 year since MICN and 1 case that were 6 months after MICN. Three patients after MICN treated with conventional medication were enrolled as controls. The evaluation indicators include corneal sensation, sub-basal corneal nerve density, tear secretion and tear break-up time, best-corrected visual acuity, corneal epithelium condition and clinical symptoms. All rhNGF group patients were treated with rhNGF eyedrops for 4 weeks.
ResultsAfter 4 weeks of rhNGF drops treatment in 5 patients, corneal sensation was significantly improved, corneal nerve fiber length, nerve branch density increased, and corneal nerve trunk diameter improved. At the same time, corneal damage was reduced and best-corrected visual acuity was improved. Both corneal sensation and nerve density, as well as corneal morphology, recovered faster compared to postoperative patients who applied conventional medications.
ConclusionsrhNGF eyedrops may speed up the healing process of corneal neurotization and help improve corneal sensation. Meanwhile, the time point of applying rhNGF eyedrops may depend on the baseline corneal nerve condition.
