Molecular mechanisms of sudden unexplained death and recent updates on molecular autopsy strategies for forensic investigation

Forensic investigation of sudden unexplained death (SUD) has long been a challenging issue due to its insidiousness and complex pathological basis. Comprehensive understanding of the cause of death in SUD cases could not only provide scientific guidance for judicial investigations, but also help to prevent such tragedies from recurring by identifying at-risk individuals. Recent development and application of molecular autopsy have provided crucial support for addressing this problem. In such a context, this review systematically collates the potential molecular mechanisms underlying SUD with a focus on its genetic background. In addition, the progress and challenges of existing molecular autopsy strategies were elaborated in detail, in order to guide the multidisciplinary management of SUD from the perspective of forensic geneticists.
