LRG1 does not reflect systemic inflammation in hidradenitis suppurativa: toward a fibrosis-associated biomarker?

Hidradenitis suppurativa (HS) is a chronic inflammatory skin disease characterized by painful nodules, abscesses, and scarring. Leucine-rich alpha-2 glycoprotein 1 (LRG1) has emerged as a biomarker of systemic inflammation and fibrosis in various immune-mediated diseases. Its role in HS, however, remains unexplored. This study aims to evaluate serum LRG1 levels in patients with HS and assess their relationship to systemic inflammatory cytokines and clinical disease features. Serum levels of LRG1, IL-6, IL-1β, IL-17A, TNF-α, and high-sensitivity C-reactive protein (hs-CRP) were measured by ELISA in 40 HS patients and 40 age- and sex-matched healthy controls (HC). Disease severity was assessed using HS Physician Global Assessment and Hurley staging. Compared to HC, HS patients had significantly higher levels of IL-6, IL-1β, IL-17A, TNF-α, and hs-CRP (p < 0.001), even in mild cases. However, LRG1 levels did not differ significantly between groups (p = 0.31) and showed no correlation with inflammatory cytokines or disease activity scores. Notably, patients with visible scarring exhibited a trend toward higher LRG1 levels (p = 0.067), suggesting a possible link to tissue remodeling or fibrosis. Despite robust systemic inflammation in HS, serum LRG1 levels remain unchanged, indicating that it may not reflect acute inflammatory activity in this condition. The observed association with scarring suggests a potential role for LRG1 in fibrotic or chronic disease phases. Further studies are warranted to explore its utility as a marker of disease progression or tissue remodeling in HS.
