Evaluation of serum inflammatory markers in acne patients treated with systemic isotretinoin or doxycycline

Acne vulgaris is a chronic, inflammatory disease of the pilosebaceous unit. This study aimed to investigate serum inflammatory markers in acne patients and compare the anti-inflammatory effects of systemic isotretinoin and doxycycline by evaluating their effects on these markers.
MedhodsThis study was designed as a retrospective study. 100 patients with moderate to severe acne vulgaris were treated with either doxycycline (100 mg/day) or isotretinoin (0.5–1 mg/kg/day) and compared with 50 age- and sex-matched healthy controls. Serum inflammatory markers, including Systemic Immune-Inflammation Index (SII), Systemic Inflammation Response Index (SIRI), Aggregated Systemic Inflammation Index (AISI), Neutrophil/Lymphocyte Ratio (NLR), Platelet/Lymphocyte Ratio (PLR), and Monocyte/HDL Ratio (MHR), were measured at baseline and during treatment.
ResultsNo significant baseline difference was found between groups. In the doxycycline group, SII, SIRI, and AISI levels significantly decreased at 2 months. In the isotretinoin group, no significant within-group changes were observed. Between-group comparisons revealed lower inflammatory marker values in the doxycycline group at 2 months.
ConclusionDoxycycline treatment led to significant short-term reductions in serum inflammatory markers, suggesting more rapid anti-inflammatory effects than isotretinoin. These findings highlight the potential utility of these indices in monitoring treatment response.
