A 16-week split-face randomized placebo controlled study on the utility of adding intradermal platelet rich plasma to 2% hydroquinone cream in the treatment of melasma

The aim of the study was to evaluate the effectiveness of Platelet rich plasma (PRP) in treatment of melasma by comparing it with placebo (normal saline) and measuring the additional benefit of PRP over hydroquinone. This was an interventional, split-face randomized placebo control trial. On one half of face platelet rich plasma and on other half placebo was injected intradermally every monthly for 4 months. Sunscreen was applied over full face and hydroquinone 2% cream was applied only over the dark patches. Melasma improvement was recorded at every visit by using Hemi- Modified Melasma area severity Index (Hemi-mMASI), Melasma Severity Index (MSI) and Investigators Global assessment scores (IGA). Dermoscopy patient global assessment score and Melasma quality-of-life score were recorded at the baseline and at end of the study. Total 45 patients completed the study with female to male ratio 1.8:1 of mean age of 29 ± 8.7 years. Hemi-mMASI decreased by 32% in the PRP group and 36% in the placebo group at the end of 16 weeks (p > 0.05). Dermoscopy in PRP group revealed excellent improvement in 20% patients and good improvement in 17% patients while the improvement in the placebo group was 15% and 13% patients, respectively. Side effects noted were mainly erythema, exacerbation of acne and pain during injection. Intradermal injection of platelet rich plasma(PRP) and saline showed comparable efficacy in decreasing melasma pigmentation and PRP did not provided any additional benefit over 2% hydroquinone.
