Postoperatives Vorhofflimmern nach kardiochirurgischen Eingriffen

Postoperative atrial fibrillation (POAF) represents the most frequent complication after cardiac surgery. While POAF was traditionally regarded as a relatively benign phenomenon, perspectives have changed considerably in recent years. Growing evidence highlights its association with postoperative morbidity, adverse long-term outcomes and last but not least, the substantially increased resource consumption in an increasingly elderly cardiac surgery population. This article first outlines the definition, incidence, etiology and clinical relevance of POAF, followed by an appraisal of predisposing risk factors and the role of pharmacological and mechanical interventional strategies for avoidance of POAF. Finally, the available treatment approaches are reviewed, with a specific focus on antiarrhythmic drugs and anticoagulation management in the context of the current literature and recommendations.
