Moderne Formen der mechanischen Kreislaufunterstützung zur Behandlung der terminalen Herzinsuffizienz

Mechanical circulatory support (MCS) has become an indispensable therapeutic option in the treatment of end-stage heart failure. This article provides a structured overview of current MCS approaches, including temporary systems, such as extracorporeal membrane oxygenation (ECMO), Impella® and CentriMag® as well as implantable long-term devices (e.g., HeartMate 3™, Berlin Heart EXCOR®). In addition to classification according to the duration of support, anatomical location, and implantation technique, the review highlights surgical aspects, clinical indications and emerging treatment concepts, such as “ECMELLA” (ECMO+Impella), hybrid biventricular assist devices (hybrid BiVAD) and minimally invasive left ventricular assist device (LVAD) implantation. Emerging systems, such as DuoCor and FlowMaker® could open up additional perspectives in the future.
