Mitral valve repair for secondary mitral regurgitation remains a clinical challenge; however, recent advances in surgical techniques offer the potential for safe and durable treatment alternatives.
ObjectiveTo evaluate the outcomes of various subannular mitral valve repair techniques in patients with ventricular secondary mitral regurgitation.
ResultsA range of subannular repair approaches has been developed for ventricular secondary mitral regurgitation. Papillary muscle approximation was evaluated in a small randomized controlled trial, which did not demonstrate clear clinical or echocardiographic superiority over standard repair methods. In contrast, papillary muscle head relocation techniques have shown promising results in a prospective single-arm registry, with encouraging clinical and echocardiographic outcomes observed at 1‑year follow-up.
ConclusionSubannular mitral valve repair techniques represent a promising, modern therapeutic option for patients with ventricular secondary mitral regurgitation, with evolving evidence supporting their efficacy.
