<p>Orthotopic heart transplantation is the gold standard for the treatment of advanced heart failure. Postoperatively, one major complication is primary graft dysfunction (PGD) which can be manifested as left ventricular failure, right ventricular failure or a&#xa0;biventricular dysfunction. Once diagnosed, treatment options comprise pharmacotherapy and in severe cases mechanical circulatory support (MCS). It is important to note that not only left ventricular impairment but also isolated right ventricular failure is of prognostic value. Hence, in this article we aim to illustrate the treatment options for primary right ventricular failure after heart transplantation based on a&#xa0;series of patient cases with reference to the complex anatomy and hemodynamics of the right heart and the treatment options available.</p>
