<p>As cardiac surgery is associated with both an increased risk of bleeding and an increased risk of thromboembolism, preoperative management of these high-risk patients is of great importance. Almost all patients are treated with anticoagulants and/or antiaggregants. In addition to laboratory diagnostics, the patient’s medical history regarding a&#xa0;pre-existing tendency to haemorrhage or thrombosis is essential. Substitution of blood products is common in cardiac surgery. Prophylactic administration is—with regard to bleeding tendency, transfusion frequency and outcome—of no significance except in exceptional cases. The focus is on therapeutic use. Various blood products are discussed with regard to their optimal use as well as their advantages and disadvantages, taking into account the special aspects of haemotherapy in cardiac surgery.</p>
