<p>Ventricular assist devices (VAD) have become established as a&#xa0;bridge to heart transplantation but also as a&#xa0;long-term treatment and possible alternative to heart transplantation for patients with advanced heart failure. Meanwhile, more than 50% of implanted devices are implanted under the strategy of long-term support. Topics such as avoiding complications, improving the quality of life, shared care, travelling with VADs, noncardiac surgery and palliative care are becoming increasingly more important. Within a&#xa0;multidisciplinary team for the care of patients with long-term mechanical circulatory support (LT-MCS) systems the VAD coordinators play a&#xa0;key role preoperatively in the education and evaluation of patients as well as perioperatively and postoperatively with a&#xa0;wide range of tasks. The knowledge and skills of the VAD coordinators are crucial to ensure optimal VAD care and to promote future technological progress. It is to be expected that with the increasing implementation of digital applications and telemonitoring there will be an expansion and changes in the tasks of the VAD coordinators. Interdisciplinary guidelines regarding long-term care, implementation of shared care programs and research projects are lacking and would be optimal to achieve further improvement in the quality of life and survival rates.</p>
