Aim <p>This study aims to systematically evaluate the quality, reliability, and user engagement of YouTube videos related to Holmium Laser Enucleation of the Prostate (HoLEP) as sources of patient education.</p> Materials and methods <p>A cross-sectional analysis of 72 publicly accessible YouTube videos was conducted using the keyword “HoLEP” (February 2025). Videos were categorized by upload source, content type, geographic origin and language accessibility. Quality and reliability assessment utilized validated instruments: the Journal of the American Medical Association (JAMA) score (0–4), Global Quality Score (GQS, 1–5), and 5-point modified DISCERN score (1–5). A statistical analysis was conducted to analyze associations between video metrics and assessment scores.</p> Results <p>Urologists (41.7%) and private hospitals (29.2%) dominated uploads, while academic institutions contributed 15.3%. Surgery-only content prevailed (54.2%), but combined surgery + information videos showed superior reliability (DISCERN:3 vs. 1, <i>p</i> = 0.005) and quality (GQS:4 vs. 1, <i>p</i> &lt; 0.001). High-reliability videos (15.3%) attracted 7.5× more likes (219 vs. 29, <i>p</i> = 0.005). DISCERN strongly correlated with engagement (likes <i>r</i> = 0.426; comments <i>r</i> = 0.400, <i>p</i> &lt; 0.01) and GQS (<i>r</i> = 0.763, <i>p</i> &lt; 0.0001), whereas JAMA scores showed no significant associations with engagement (<i>p</i> &gt; 0.05).</p> Conclusion <p>Despite clinicians and private hospitals producing most HoLEP-related YouTube content, academic centers contributed a disproportionate share of the most reliable videos. Multimodal formats that combine operative demonstrations with clear, patient-oriented information achieved higher reliability and quality scores than surgery-only videos, whereas high-quality patient narratives remained limited. These findings underscore the need for professional societies and content creators to prioritize accessible, evidence-based educational materials and for platforms to enhance the visibility of verified surgical videos.</p>
