Purpose <p>To compare efficacy and safety of standard Holmium: YAG Cyber Ho generator vs. Cyber Ho with Magneto technology (Quanta System<sup>®</sup>) in mininvasive percutaneous nephrolithotripsy (Mini-PCNL) of renal stones between 10 and 20&#xa0;mm.</p> Materials and methods <p>Patients underwent Mini-PCNL using standard Cyber Ho generator (Group A) versus Cyber Ho with Magneto technology (Group B). Patients from Group B were prospectively followed and retrospectively compared to patients from Group A. All procedures were performed in supine position. Pneumatic balloon dilator with an 18 Fr access sheath, 12 Fr nephroscope and 800&#xa0;μm laser fiber were used in all cases. Energy and frequency settings were adapted to stone features in each patient. Perioperative parameters, stone-free rate (SFR) and postoperative complications were assessed.</p> Results <p>100 patients were enrolled (50 in each group). Preoperative features were comparable. Mean stone size was 17.3&#xa0;mm in Group A and 16.7&#xa0;mm in Group B (<i>p</i> = 0.21). Mean operative time was 50.3 vs. 44.5&#xa0;min (<i>p</i> = 0.09), with a mean laser time 19.5 vs. 12.2&#xa0;min (<i>p</i> = 0.04), delivered energy 5.9 vs. 4.1 KJ (<i>p</i> = 0.04) and ablation speed 0.6 vs. 0.9 mm<sup>3</sup>/s (<i>p</i> = 0.04) in Group A vs. B, respectively. In Group A the use of a basket to retrieve fragments was significantly more frequent. SFR was 82.0% with the standard Cyber Ho and 94.0% with the Magneto technology (<i>p</i> = 0.05). Complication rate was comparable.</p> Conclusions <p>The implementation of the Holmium: YAG Cyber Ho laser generator with the new Magneto technology provides a significant advantage in terms of dusting efficacy and complete stone clearance compared to the standard Cyber Ho generator in the mininvasive percutaneous treatment of renal stones between 10 and 20&#xa0;mm.</p>

Conclusions
