Initial clinical experience with 6.3 Fr single‑use flexible ureteroscope versus 8.5 Fr in retrograde intrarenal surgery with flexible and navigable suction ureteral access sheath of variable diameters: a single‑center comparative study

This study combined flexible and navigable suction ureteral access sheaths (FANS) to compare the efficacy and safety of the newly emerged 6.3Fr flexible ureteroscope (fURS) and 8.5Fr f-URS in the treatment of kidney stones.
MethodsThis is a single-center retrospective study aimed at comparing the safety and efficacy of 67 patients who received 6.3Fr or 8.5Fr fURS for upper urinary tract stone management. The two groups were respectively the 6.3Fr group (n = 19) and the 8.5Fr group (n = 48). The primary outcomes were the immediate and one-month stone-free rates (SFR). Secondary outcome included operation time and complications.
ResultsBaseline characteristics between two groups were similar. No significant difference was noted in immediate SFR (89.5% vs. 83.3%, p = 0.798) and one-month SFR (89.5% vs. 85.4%%, p = 0.967). 3 cases in 6.3Fr group and 7 cases in 8.5Fr group resulted in failure of 12/14Fr FANS insertion, respectively. In cases where a 12/14Fr FANS could not be inserted, the 11/13Fr and 10/12Fr FANS would be attempted. 10/12Fr FANS inserting rate in 6.3Fr group was higher than in 8.5Fr group (100% vs. 0%, p = 0.036). Operation time in 6.3Fr group was significantly shorter than 8.5Fr group (36.32 ± 19.23 min vs. 48.48 ± 16.34 min, p = 0.011). No severe complications were observed in either group, with only one case of postoperative fever in 8.5 Fr group.
ConclusionsRIRS with 6.3Fr fURS yielded comparable SFR compared to those with 8.5Fr fURS and the 6.3Fr group demonstrated shorter operation time, with low complications.
