Rethinking calcaneal spurs: a biomechanical approach to heel pain

The calcaneal spur is often implicated in plantar fasciitis with associated heel pain; however, this perspective may overlook critical biomechanical factors. This viewpoint challenges conventional treatment paradigms that focus solely on symptomatic relief through interventions, such as shockwave therapy or corticosteroid injections, which lack robust evidence for efficacy. Instead, it emphasizes the importance of addressing gait patterns and foot biomechanics, particularly in cases with pronation or supination syndromes, where the plantar fascia may exhibit excessive laxity or rigidity. Modifying ambulation through footwear adjustments and tailored advice represents a promising avenue for managing this condition.
