Short-term effects of adding ilium mobilization with movement to hip and knee focused exercises in patients with patellofemoral pain and foot pronation

Patellofemoral pain (PFP) is a frequent musculoskeletal condition that affects young adults and hinders their functional daily activities. This study aims to evaluate the short-term effects of incorporating ilium mobilization with movement (IMWM) into hip-knee focused exercises (HKFEs) on pain and function, hip and knee muscle strength, frontal knee projection angle (FKPA), and pelvic tilt angle (PTA) in patients with PFP and pronated foot.
Material and methodsThis double-blinded randomized controlled trial (RCT) was conducted on 36 patients, 18–35 years old who were diagnosed with unilateral PFP and foot pronation. Patients were randomly allocated into two equal groups: the study group received IMWM with HKFEs and the control group received (HKFEs) only. Treatment was performed in both groups for 4 weeks. Primary outcomes were knee pain assessed by numeric rating pain scale (NRPS) and function by anterior knee pain scale (AKPS). Secondary outcomes evaluated hip and knee muscle strength, FKPA and PTA by handheld dynamotor, photogrammetry and pelvic inclinometer, respectively. The data were analyzed at baseline and 4 weeks after interventions.
ResultsThere were statistically significant differences in pain, function, hip extensor strength and PTA (p = 0.001) between both groups, in favor of the study group; however, hip abductor, external rotator muscle strength, knee extensor muscle strength or FKPA showed no statistically significant differences (p > 0.05) between groups.
ConclusionAdding IMWM to HKFEs results in significant short-term improvements in knee pain and function, also helping the pelvis realignment by increasing hip extensor strength in patients with PFP and foot pronation.
