摘要

Background

Since 2020, physicians and psychotherapists in Germany can prescribe digital health applications for mental disorders (PsyDiGA). The numbers of prescriptions and utilization have remained below expectations. To date, no studies have systematically compared the perspectives of patients and psychotherapists.

Objective

Investigation of the potentials and limitations of PsyDiGA from the perspectives of psychotherapists as prescribers and patients as users.

Methods

A nationwide survey among psychotherapists was conducted using questionnaires, complemented by qualitative telephone interviews with patients and psychotherapists. The data were analyzed descriptively (quantitative) and using thematic content analysis (qualitative) according to Mayring.

Results

Questionnaires were completed by 321 psychotherapists and interviews were conducted with 18 patients and 16 psychotherapists. Both groups expressed ambivalence towards PsyDiGA. They identified advantages, such as temporal and spatial availability, the possibility of independent use and the relief of the strained treatment situation with limited places in outpatient psychotherapy. Concerns were raised about data protection, technical barriers, lack of individualization and the loss of the therapeutic relationship. Psychotherapists emphasized the lack of individualization, while patients highlighted the lack of personal contact.

Conclusion

The implementation of PsyDiGA depends less on technological than on interpersonal, structural, and trust-related factors. A sustainable integration into psychotherapeutic care requires designing technical innovations that complement rather than replace the therapeutic process.
