Posttraumatic stress disorder (PTSD) is a frequent disorder with a lifetime prevalence of approximately 4% that is associated with a significant impairment in the quality of life. It is particularly prevalent in clinical samples. Although there is a high rate of spontaneous remission within the first 6 months, PTSD tends to become chronic without treatment, making therapy necessary. Psychotherapy is recommended before pharmacotherapy.
ObjectiveThe most effective psychotherapy programs for the treatment of PTSD, complex PTSD and the special features of comorbid mental disorders are described.
Material and methodsThe results from meta-analyses, important original studies and the recommendations of national and international treatment guidelines are taken into account.
Results and conclusionTrauma-focused psychotherapy is the first line treatment for PTSD. It has a high acceptance and demonstrates large effect sizes. The best outcomes have been found for prolonged exposure, cognitive processing therapy and eye movement desensitization and reprocessing. These therapies either focus primarily on changing the appraisal of the traumatic event and its consequences or more strongly on exposure to the memory or they combine both interventions. Trauma-focused therapy should also be used in patients with complex PTSD and comorbid mental disorders, such as psychosis and substance use disorders.
