Aufmerksamkeitsdefizit-/Hyperaktivitätsstörung und suizidales Verhalten
摘要
Multiple studies and review articles have shown an association between attention deficit hyperactivity disorder (ADHD) and suicidal behaviour with a consequently high relevance for the diagnostic assessment and therapeutic approach in the treatment of affected youths.
ObjectivesThe aim of this article was to summarize the findings of a current literature search, to describe explanatory models for the correlation between ADHD and suicidal behaviour and to derive diagnostic, psychotherapeutic and psychiatric implications for clinical practice.
Materials and methodsBased on a selective review of recent literature, the following article describes the association between ADHD and suicidal behaviour, presents an illustrative case report and includes own practical observations and experience.
ResultsChildren and adolescents with ADHD show higher rates of suicidal ideations, suicide attempts and suicides in comparison to healthy controls, with a 3-fold, 2‑fold and up to 6‑fold higher risk, respectively. Conversely, a significant proportion of suicidal youth populations meets the criteria for ADHD. Risk factors for suicidal behaviour among ADHD patients are high impulsivity, dysfunctional emotional regulation, psychiatric comorbidities, negative interpersonal interactions, low self-esteem and feelings of being overwhelmed.
ConclusionsAn early diagnosis and appropriate treatment, a focused anamnesis for suicidal behaviour and the integration of therapeutic strategies targeting self-efficacy as well as suicide prevention, while taking into account disorder-specific obstacles, are essential in ADHD treatment.
