Evaluating Rectus Abdominis Fascia and Alloderm as Wrapping Materials in Diced Cartilage Grafts for Dorsal Nasal Augmentation

Dorsal nasal augmentation can be performed using autologous diced costal cartilage wrapped in various materials. This study aimed to evaluate the outcomes of two different wrapping materials—rectus abdominis fascia and Alloderm—on postoperative aesthetics, functionality, and donor-site morbidity.
MethodsThis is a retrospective study that included patients who underwent dorsal nasal augmentation. Patients were grouped into two cohorts: the rectus abdominis fascia group and the Alloderm group. Data collected included demographic information, surgical parameters, and postoperative outcomes. Anthropometric analyses of pre- and postoperative nasal dimensions were performed using lateral and frontal profile photographs. Additionally, subjective satisfaction was assessed using the rhinoplasty outcomes evaluation (ROE) questionnaire and visual analog scale (VAS), while objective outcomes were rated by blinded surgeons.
ResultsBoth groups demonstrated significant improvements in nasal projection and contour, with similar anthropometric changes. The Alloderm group showed better scar outcomes and fewer complications, such as palpable graft particles. Postoperative recovery time at the chest donor site was significantly longer in the fascia group, with increased incision length and drainage compared to the Alloderm group. Both groups achieved substantial aesthetic and functional improvements, but the Alloderm group exhibited higher patient satisfaction and better surgeon-rated aesthetic outcomes at both short- and long-term follow-up.
ConclusionsBoth rectus abdominis fascia and Alloderm are effective in achieving aesthetic improvements in dorsal nasal augmentation, with Alloderm showing advantages in terms of donor-site morbidity. The Alloderm technique may be preferable for minimizing complications and enhancing long-term aesthetic outcomes.
