<p>Segmental colitis associated with diverticulosis (SCAD) is an under-recognized clinical entity characterized by chronic mucosal inflammation localized to a colonic segment affected by diverticular disease. Despite its increasing recognition, it remains frequently misdiagnosed due to clinical and radiographic overlap with acute diverticulitis and inflammatory bowel disease (IBD). This review explores the pathogenesis, clinical presentation, and imaging features of SCAD. We highlight the critical endoscopic and radiologic markers that differentiate SCAD from its mimics and emphasize the necessity of a multidisciplinary approach for optimal patient management.</p>

Segmental colitis associated with diverticulosis (SCAD) is an under-recognized clinical entity characterized by chronic mucosal inflammation localized to a colonic segment affected by diverticular disease.
