The MIBI is dead, long live the MIBI: revisiting [99mTc]Tc-Sestamibi thyroid scintigraphy in the era of ultrasound, cytology, molecular diagnostic, and 18 F-FDG PET/CT

[99mTc]Tc-sestamibi (MIBI) thyroid scintigraphy has experienced fluctuating clinical relevance, overshadowed by the rise of ultrasound, cytology, molecular diagnostics, and FDG PET/CT. However, recent evidence highlights a reinvigorated role for MIBI in specific clinical niches, notably in indeterminate thyroid nodules and iodine-induced thyroid dysfunctions. Our opinion paper synthetizes the biological background of thyroid imaging with MIBI, including semi-quantitative analysis. Current clinical indications and clinical results are discussed taking into account the emerging role of 18 F-FDG PET/CT in the field of cytologically indeterminate thyroid nodules offering a balanced perspective on integrating MIBI into modern thyroid diagnostics.
