Complications and outcomes of radioligand therapy in patients with neuroendocrine neoplasms depending on the baseline glomerular filtration rate

Neuroendocrine neoplasms (NENs) represent a rare group of neoplasms originating from neuroendocrine cells in almost any part of the body. One of subsequent line of treatment is radioligand therapy (RLT) with use of beta-emitting isotopes. The most common adverse events are deterioration of kidney and bone marrow function. The aim of the study was to assess the differences in complications and outcomes of RLT between patients with initially low and normal renal function.
MethodsStudy group included 127 patients with progressive NENs who underwent one cycle of RLT. Renal, bone marrow, glucose, and chromogranin A were assessed before Course I, Course IV and in follow-up. A representative subgroup of 44 patients reported for long-term visit.
ResultsRLT did not cause significant decrease of GFR during the treatment, however over 6% GFR decrease was observed in long-term follow-up. Blood morphology parameters significantly decreased during the treatment, and remained decreased in long-term observation. Glucose concentration did not change during the treatment, though an increase of 12 mg/dL in long-term observation was noted. Patients with initially lower GFR had improvement in GFR values. Calculated PFS and OS probability at 60 months after treatment were 41.5% and 74% respectively.
ConclusionsRLT cause a statistically but not clinically significant decrease of all blood morphology parameters. In long-term observation after RLT a decrease of GFR and increase in glucose concentration are observed. Initially low GFR seems not to limit the possibility of RLT, and even GFR increase in this group of patients is possible.
