Background <p>Over the past few decades, there has been a significant rise in the demand for body contouring procedures. There is an increase in the number of patients presenting after bariatric surgery, massive weight loss or multiple pregnancies with the complaint of laxity of the lower body and lack of an adequate shape and contour.We aimed to evaluate the effectiveness and safety of a novel lower body lift technique utilizing de-epithelialized dermal flaps, compared to the traditional method, in patients seeking body contouring.</p> Methods <p>This is a retrospective review that was conducted on 76 female patients from January 2018 to December 2024. Patients were divided into two groups; those that underwent the modified technique with de-epithelialized flaps and those that recieved the conventional approach. Data collected included demographics, complications, and patient satisfaction measured via the BODY-Q questionnaire. Surgical steps including the preoperative markings are discussed.</p> Results <p>The study demonstrated a lower complication rate in the modified group (17%) compared to the traditional group (50%), with reductions in seromas, wound healing issues and long-term scar deformities. No recurrences of ptosis were observed in the follow-up period in the study group. Patient-reported outcomes favored the modified technique, with higher satisfaction scores, improved body image, and enhanced psychological well-being.</p> Conclusion <p>The use of de-epithelialized dermal flaps in lower body lifts offers a promising, effective approach for achieving durable aesthetic results while minimizing complications.</p> Level of evidence <p>Level III, risk / prognostic study.</p>

Background
