Hinge maneuver and pivot point: the nasomaxillary articulation as a natural osteotomy in modern rhinoplasty

Primary rhinoplasty presents a challenge in the management of the bony nasal third, especially in cases of deviation or asymmetry where osteotomies are required.
MethodsA retrospective observational study was conducted on all patients operated between May 2024 and May 2025 using an approach where paramedian osteotomies were avoided and instead utilizing the nasomaxillary articulation as a natural hinge point allowing mobilization of the nasal bones while preserving structural stability, generating a natural shadow along the lateral nasal wall, and optimizing in this way the dorsal aesthetic lines. Additionally, a comparative analysis was performed with a control group of patients operated on using traditional paramedian osteotomies. Standardized photographic documentation was used to assess bony symmetry and dorsal aesthetic lines. Objective comparison was complemented by a patient satisfaction survey using a validated 1–5 Likert scale.
ResultsA total of 154 patients (108 females, 46 males) were included. The mean age was 28.7 years (range: 18–54). One hundred twelve cases were primary rhinoplasties, while 42 were secondary procedures. Mean operative time was 69 min while the mean follow-up was 13.2 months. Satisfaction scores were higher when compared to the traditional technique (4.7 vs. 3.9).
ConclusionsThis approach proposes an anatomically conservative, yet effective technique for the management of the bony nasal third. Results demonstrated a great improvement in nasal symmetry and a more predictable postoperative recovery.
