The external post-mortem examination is a central but error-prone instrument for determining the cause of death. Based on two cases, it is shown that deaths initially classified as natural, with presumed esophageal variceal bleeding, were identified as nonnatural (suspected suicide) during the second post-mortem examination. Both initial assessments were based on medical misjudgements despite obvious findings. The results highlight the need for qualified training, an open culture of error reporting and mandatory feedback in cases of misjudgement. Furthermore, the importance of the second post-mortem examination prior to cremation for ensuring legal and diagnostic accuracy is emphasized.
