<p>Unintended perioperative hypothermia remains one of the most common preventable complications in surgical patients despite advances in anesthesia and warming strategies. Even mild reductions in the core body temperature can adversely affect coagulation, wound healing, immune function and patient comfort. The updated German S3 guidelines on the prevention of perioperative hypothermia provide evidence-based recommendations for temperature management throughout the entire perioperative care pathway. At the same time, recent studies, particularly the PROTECT trial, have led to a&#xa0;more differentiated view of perioperative temperature targets. This review article summarizes the physiological principles of thermoregulation, the clinical consequences of perioperative hypothermia and current recommendations regarding temperature monitoring, preoperative prewarming, intraoperative thermal management and postoperative prevention and treatment of shivering. The recommendations of the S3 guidelines serve as the conceptual framework for integrating temperature management into modern perioperative care pathways.</p>
