The rising survival rates following breast and gynecologic cancers have led to an increasing number of young patients expressing a desire to have children. After cancer treatment, pregnancy does not negatively impact maternal prognosis. However, achieving pregnancy after cancer remains a clinical challenge. On the one hand, fertility may be postponed to a later stage in life and be significantly impaired by the treatment with gonadotoxic agents; and on the other hand, potential long-term effects of cancer therapy must be considered. Therefore, all patients of reproductive age should receive professional counseling before the start of treatment regarding possible impacts on fertility and available options for fertility preservation. In cases of interest in fertility-preserving therapies, early referral to a specialized center is essential.
