Endometrial carcinoma (EC) is the most frequent pelvic malignancy in women and has an annual incidence of 11,000 newly diagnosed cases in Germany. The prognosis of EC is generally good as more than two thirds of the cases are diagnosed in an early stage of the disease. Established risk factors for the development of EC are obesity, diabetes mellitus, estrogen-containing hormone treatment, tamoxifen treatment, polycystic ovary syndrome (PCOS) and hereditary tumor syndromes, such as mutations in the DNA repair genes MLH1, MSH2, MSH6 and PMS2, which result in hereditary nonpolyposis colorectal cancer (HNPCC). Important for the counselling of women with an elevated risk for EC is information on risk-reducing interventions, such as physical activity, bariatric surgery, intrauterine pessaries and endometrial ablation, a surgical method for treatment of uterine bleeding disorders. In rare cases, e.g., women with known HNPCC-associated mutations, a prophylactic hysterectomy is also an option.
