The prediction and prevention of type 1 diabetes form the framework for the discussion as to whether antibody screening should become part of standard care. The islet autoantibody test does not characterize the metabolic situation but is a snapshot of autoimmunity and enables a prognosis of the future risk of type 1 diabetes; however, for people with a positive test it does not provide a reliable, individualized prediction of when the disease will occur. Drug treatment for prevention of type 1 diabetes is not yet available. A positive test therefore confronts the affected person with a disease prognosis without any meaningful treatment being available. The psychological burden of the test can therefore be considerable. Further clinical studies are required to investigate the benefits and risks of the islet autoantibody test.
