<p>The operating room is a&#xa0;high-risk environment where patient safety is determined by the interplay of people, technology and organization (MTO). This article describes a&#xa0;practice-oriented safety model based on the MTO concept and presents concrete measures for error prevention in everyday perioperative practice. Particular focus is placed on structured Stage-Gate processes, team communication, safety culture and the role of modern digitalization and technical assistance systems.</p>
