A comparative study of minimally invasive plate osteosynthesis with t-shaped incision technique and standard volar approach with pronator quadratus preservation in volar plating of distal radius fractures

This study introduces a novel minimally invasive plate osteosynthesis technique using two small T-shaped incision (T-MIPO) for the treatment of distal radius fractures. It compares the short-term postoperative pain levels, assessed by the visual analogue scale (VAS), between T-MIPO and the standard volar approach (SVA) with preservation of the pronator quadratus (PQ).
MethodsThe retrospective study enrolled 65 patients with distal radius fractures, divided into two groups: the T-MIPO group (n = 36) and the SVA with PQ preservation group (n = 29). The choice of surgical technique (T-MIPO or SVA with PQ preservation) was determined by the same medical team, and all procedures were performed by a single orthopedic surgeon. Postoperative pain was assessed using the VAS at 1 and 3 days following the surgery.
ResultsThis retrospective study demonstrated a significant difference in postoperative pain VAS score between the groups on day 1 (T-MIPO: 2.11 ± 0.57 vs. SVA with PQ preservation: 3.66 ± 0.77, p < 0.001) and day 3 (T-MIPO: 0.97 ± 0.65 vs. SVA with PQ preservation: 2.34 ± 0.77, p < 0.001). However, the degree of VAS score reduction between postoperative day 1 day 3 was not significantly different between the two groups.
ConclusionsThis underscores the advantages of T-MIPO technique for managing distal radius fractures. This approach achieves radiological outcomes comparable to the SVA with PQ preservation technique while significantly reducing postoperative pain on both day 1 and day 3. Additionally, the T-MIPO technique offers benefits such as minimizing soft tissue damage and providing superior cosmetic benefits results.
