Out-of-hospital cardiac arrest (OHCA) continues to pose a significant clinical challenge with low survival rates. The updated 2025 guidelines emphasize the critical role of the chain of survival, optimized basic life support (BLS) and the strategic use of extracorporeal cardiopulmonary resuscitation (ECPR). Following the return of spontaneous circulation (ROSC) the clinical course is predominantly shaped by the post-cardiac arrest syndrome (PCAS). Structured care within specialized centers and a multimodal timely neuroprognostication are essential to improve neurological outcomes and facilitate evidence-based decision-making regarding further treatment.
