<p>The treatment of anal fistulas involving the sphincter muscles represents a&#xa0;challenge for proctological surgeons. Both the genesis and the exact course of the fistula are crucial for the outcome. Numerous surgical methods have been established in recent decades; these are divided into sphincter-sparing, plastic reconstruction and sphincter-cutting procedures with subsequent reconstruction. The surgical methods mentioned are subject to the tension between achieving the highest possible healing rate but also not endangering the patient’s continence performance. In this article, the sphincter-sparing ligation of intersphincteric fistula tract (LIFT) method is presented step by step and illustrated with images taken intraoperatively. In addition, the perioperative management, our own results and the current literature are discussed.</p>
